You Must Print Out these Forms and Include them With your console.

 If you do not Include them Your order May be Extremely Delayed or Canceled.

Follow these Steps:

  1. Go to the Store, Fill out and Purchase the Send in Service HERE
  2. Once Paid for you will Get a Email from us Saying the order was Placed.
  3. You will Need to Write down the Order Number On the Form. This is a Very Important step DO NOT FORGET THIS!
  4. The Order Number is a 5 Digit Order Number and it will be from us.You can Get your Order Number from your Email
  5. Now fill this Form Out:
